Job Title: PATCH Nurse (Paediatric Assessment and Treatment Contribution at Home)

Job Summary:

Join our community-facing pediatric nursing team to provide high-quality acute clinical care to children within their home environment. You will work closely with emergency departments and pediatric wards to facilitate safe, early hospital discharges and prevent unnecessary readmissions.

Responsibilities:

• Provide skilled clinical interventions, including medication administration, wound care, and comprehensive physical assessments for children at home.

• Act as a direct point of contact for families, offering expert clinical advice, safeguarding oversight, and health education to manage illness confidently.

• Collaborate closely with multidisciplinary hospital teams and community GPs to coordinate smooth referral pathways and care plans.

Qualifications:

• Active registration as a Registered Children’s Nurse (RCN / RN Child) with the Nursing and Midwifery Council (NMC).

• Proven post-registration clinical experience within acute pediatric settings, community nursing, or complex pediatric care.
